In short
Flushing has one of the largest concentrations of older adults with Medicaid in Queens, in the towers around Main Street and in the two-family houses toward Murray Hill where a parent often lives with an adult child's family. The home care process here is the statewide one, but the practical issue is language. The NYIA program's FAQ says it provides interpreting in any language during the assessment appointments if you tell them when you schedule. Mandarin, Cantonese, and Korean are the most common requests from this ZIP cluster, and the same interpreter rule applies to NY Connects, New York Medicaid Choice, and HRA.
ZIP orientation
Approximately 11354 (downtown and the north side), 11355 (south toward Kissena Park), and 11358 (Murray Hill and Auburndale). Verify with USPS. County: Queens.
Reaching NY Connects and HIICAP from here
NY Connects, 1-800-342-9871 (Relay 7-1-1), and say "Queens." Ask for your language at the start of the call. HIICAP, 1-800-701-0501, is the Medicare counseling line; say "Queens." The nearest in-person NYC Aging HIICAP site is the RAICES / Corona Senior Center at 102-47 43rd Avenue, 718-458-7259, by appointment, a few stops west on the 7 train from Main Street.
Medicaid channel tip
Flushing Medicaid is HRA, citywide: 1-888-692-6116. Adults 65 and older and people on Medicare apply through HRA, not through the marketplace. The many storefront "Medicaid help" signs along Main Street are private businesses; the official channel is HRA and its office list on nyc.gov/hra.
What usually happens next
- Confirm Medicaid. An active Medicaid number is the first thing NYIA asks for.
- Call NYIA, 1-855-222-8350 (TTY 1-888-329-1541), and request the interpreter language at scheduling. In-person or video is usually offered.
- Nurse assessment, then clinical appointment. The FAQ says to have an identification card, a medication list, and recent medical records ready. A family member can be present.
- Outcome letter. It states whether the person meets the requirements to request personal care or consumer directed services, or to enroll in a Managed Long Term Care plan, and explains conference and fair hearing rights.
- New York Medicaid Choice (1-800-505-5678, long term care 1-888-401-6582) for plan selection if MLTC is the route. Adults 21 and older with Medicare and Medicaid who need more than 120 days of services usually must enroll in MLTC to receive them.
- Plan assessment and start of services. The plan writes the person-centered service plan; this site cannot predict hours.
Common mistakes here
- Letting the adult child translate the nurse's questions. The assessment asks the person about bathing, dressing, and moving around the home. When a relative summarizes, details get lost. Request the interpreter when scheduling and let the relative sit in as support.
- Assuming the Main Street clinic that takes the Medicaid plan will "arrange the aide." Clinics can provide records and a doctor's input, but the assessment is done by NYIA's own nurse and clinician.
- Overlooking help for a parent who is not on Medicaid. Many Flushing households support a parent whose income sits just above the Medicaid line. NYC Aging's in-home services for residents 60 and older are contribution-based; Aging Connect is 212-244-6469.
Nearby orientation
Orientation only: NewYork-Presbyterian Queens on Main Street and Flushing Hospital Medical Center on Parsons Boulevard. Also for orientation, the Department of Health's Social Care Network page lists ZIP 11355 among a handful of Queens ZIPs served in partnership between two regional networks; a Medicaid member there can still be screened through the plan or provider like anyone else.
