In short
Bayside is the part of Queens that feels like Nassau: single-family houses, driveways, the Long Island Rail Road instead of the subway, and a large Korean- and Chinese-speaking community alongside long-time Irish, Italian, and Greek households. Two things follow. First, the county is still Queens, and the programs are still the city's and the State's. Second, distance matters. The nearest in-person counseling site is a long bus ride away, so the NYIA program's video appointment option and the phone lines carry more of the load here than in the western part of the borough.
ZIP orientation
Approximately 11360 (Bay Terrace side), 11361 (central Bayside), and 11364 (Oakland Gardens). Verify with USPS. County: Queens, not Nassau, even where the street grid stops looking like the city.
Reaching NY Connects and HIICAP from here
NY Connects, 1-800-342-9871 (Relay 7-1-1); say "Queens." NYC Aging runs the city's NY Connects, which its page describes as serving people of all ages and income levels. HIICAP, 1-800-701-0501, for free Medicare counseling; say "Queens." The nearest in-person NYC Aging HIICAP site on the citywide listing is India Home in Jamaica (917-288-7600, by appointment), reached by the Q27 or Q31 toward Hillside Avenue; most Bayside residents do it by phone instead.
Medicaid channel tip
Medicaid is HRA, citywide, 1-888-692-6116. There is no northeast Queens Medicaid office. Adults 65 and older and people on Medicare apply through HRA, and locations are on nyc.gov/hra.
What usually happens next
- Medicaid active. Confirm with HRA before calling the assessor.
- Call NYIA, 1-855-222-8350 (TTY 1-888-329-1541). Ask about a video visit if travel or scheduling a home visit is difficult. The FAQ says you need a smartphone, tablet, or laptop with a camera and Wi-Fi, or a relative present who has one, plus an email address or smartphone to receive the link.
- Nurse assessment, then clinical appointment, by video or in person at home. Have an identification card, a medication list, and recent medical records at hand.
- Outcome letter stating whether the person meets the requirements to request personal care or consumer directed services, or to enroll in MLTC.
- New York Medicaid Choice (1-800-505-5678, long term care 1-888-401-6582) for plan selection if MLTC is the route. Ask which plans list Queens County; most do, but confirm.
- The plan's own visit and care plan, then services usually begin after the enrollment date. Hours are the plan's decision.
Common mistakes here
- Telling the scheduler "Nassau" because the mailing address says Bayside, NY. The county is Queens. Getting it wrong routes the call to the wrong district.
- Skipping the video option because "the nurse should see the house." The FAQ says video visits cover the same ground. If a home visit would delay things and a relative can bring a tablet, ask about video.
- A homeowner assuming the house rules out help. NYC Aging's in-home services for residents 60 and older who are not on Medicaid do not turn on homeownership. The contribution depends on income. Aging Connect is 212-244-6469.
Nearby orientation
Orientation only: NewYork-Presbyterian Queens and Flushing Hospital Medical Center, both in Flushing, are the closest large hospitals on the citywide list and the ones Bayside residents most often come home from.
