Who we are
nyhealthcareguide.com is an educational website operated by a private organization. We are not a government agency, not the New York Independent Assessor Program, not New York Medicaid, not HRA, not NYC Aging, not New York Medicaid Choice, and not a home care agency or health plan.
What we collect
If you use the summary tool at /tools/home-care-help or a contact form, we may collect your name, phone, email, ZIP, and the answers you provide about Medicaid status, age range, Medicare status, and where you are in the process. We also collect basic technical data (such as browser type, pages viewed, IP address, and approximate location) to run and improve the site.
We use Google Analytics and Vercel Analytics to understand how the site is used. These tools may set cookies or collect device identifiers. We do not ask for, and you should not enter, Medicaid or Medicare ID numbers, Social Security numbers, diagnoses, medications, or assessment results.
How we use it
We use your information to send the plain-language summary you requested and related educational information about home care paths in New York. We do not use this site to request, approve, or arrange home care, to schedule assessments, or to enroll anyone in a plan. Submissions are never sent to any health plan, home care agency, fiscal intermediary, or contractor, and nobody is paid for them.
If you provide a phone number and check the phone/text consent box, you agree that we may call or text you at that number, including with automated technology. Consent is not a condition of receiving educational content. Message and data rates may apply. You can withdraw phone consent at any time by replying STOP to a text or telling a caller you want no more outreach.
How we hold leads
Form submissions are held first-party by the site operator for follow-up and service. We do not sell your information and do not share it as a lead list. We may share data with service providers who host the site, store submissions, send messages, or support operations, under agreements that limit their use.
Your choices
You can ask us not to contact you further by emailing hello@nyhealthcareguide.com or by telling a caller you want no more outreach. You can also ask what information we hold about you and request that it be deleted. For free help with long-term services and supports, call NY Connects at 1-800-342-9871. Official programs
Decisions about Medicaid home care are made only by the New York Independent Assessor Program, your plan, HRA or your county, and the State. Always confirm rules on health.ny.gov, nyconnects.ny.gov, or with a qualified counselor.
Children
This site is intended for adults. We do not knowingly collect information from anyone under 18.
Changes
We may update this policy. The effective date and version above will change when we do. Continued use of the site after an update means you accept the revised policy.
